India is part of bakers dozen now - UN report raises air safety concerns in India

2006 : 

UN - ICAO audit report finds fault with India's safety oversight.  India's aviation ministry then under Praful Patel and the regulator under DGCA Kanu Gohain junks it and tells us all is well.
